What to do next?

Get in touch with us today to book a date. Delivering Dental will drop off medical history and consent forms to the childcare centre approximately one month before our planned dental visit. There is also an online form available to download and return to your childcare centre at least one week before our visit.

We ask for medical history and consent forms to be signed and returned to us no later than one week prior to the visit so that we can check children’s CDBS eligibility. If any children are not eligible, we will arrange for alternative payment. 

On the day, we require a small room or part of a classroom to set up (with a power point very close by). We will bring all our equipment and do not need any assistance setting up. 

Parents and guardians are welcome to attend their child’s dental check up, however it is not essential or a requirement. If parents would like to attend they can let us know on the consent form and we will organise an appointment time that suits while we’re at your facility. 

While we’re treating children we would love help from the childcare providers to bring the children to us in groups and make them feel comfortable.

We will make plans to return every 6-12 months. This ensures the children’s continued oral health and gets them steadily more comfortable with the dentist!
