Dear Parent/Guardian,

This is the medical and general consent form that allows Delivering Dental to treat your child when they are coming to your Childcare Centre.

Delivering Dental provides preventative services including check ups, cleans, remineralisation application (calcium), where needed.

The service is free for eligible families under the Federal Government’s Child Dental Benefit Scheme (CDBS). The CDBS provides eligible children between 0-17 years old with $1052 of funding for dental services every 2 years. If you receive the Family Tax Benefit A, you are likely eligible.  Delivering Dental will also confirm your eligibility for you.

If your child is not eligible for the CDBS, Delivering Dental will contact you to let you know. The alternative option is their $75 flat fee which is payable in full before the visit. This fee can be claimed back through most private health insurance providers.

Having this service at your centre, in a familiar environment, makes your child comfortable and gives them a positive dental experience. It also removes the hassle of taking your child to the dentist for their check and clean.

Your child will receive a dental gift pack after their appointment.

After their visit with us, Delivering Dental will email you a Dental Report and Invoice.

We highly recommend this program as maintaining your child’s general and oral health is extremely important to us.
